発表タイトル Switch effects of aflibercept in the cases of recurrence or tolerance to ranibizumab for exudative age-related macular degeneration

概要 [Purpose] To evaluate the switch effects of single intravitreal aflibercept injection (IVA) in the cases of recurrence after intravitreal ranibizumab injection (IVR) or tolerance to IVR for exudative age-related macular degeneration (AMD).
[Methods] Subjects comprised 52 patients (53 eyes) switched to IVA from IVR because of recurrence or tolerance in the cases treated by IVR at Kansai Medical University Hirakata Hospital between January and March, 2013. There were 39 male and 13 female. Mean patient age was 73.7 years (57-89 years). Mean number of past IVR was 7.4 times (1-35 times). There were 43 eyes of recurrence and 10 eyes of tolerance, and 28 eyes with typical AMD (t-AMD), 24 eyes with polypoidal choroidal vasculopathy (PCV) and 1 eye with retinal angiomatous proliferations. In these cases, we evaluated changes of exudative findings using optical coherence tomography and visual acuity at 1 month after first IVA, retrospectively.
[Results] Serous retinal detachment and retinal edema were observed in 52 eyes (98.1%) before IVA. These findings disappeared in 39 eyes (75.0%) at 1 month after IVA. The ratio of dry macula was 67.9% in t-AMD and 82.6% in PCV. LogMAR visual acuity improved or maintained in 51 eyes (96.2%). There was no significant difference in mean decimal visual acuity between before and after IVA.
[Conclusion] IVA can be expected an effective anatomical switch effect in the short term in the cases of recurrence and tolerance to IVR for AMD.
