概要 [PURPOSE]
Angioid streaks (AS) is caused by cracks in the collagenous and elastic layers of Bruch’s membrane. New streaks formation with subretinal hemorrhages (SRH) after ocular blunt trauma have been reported in eyes affected by AS. We report a case of AS with the formation of new streaks with SRH in both eyes without blunt trauma.
[PATIENTS]
A 59-year old man was admitted to our clinic complaining that the vision in his left eye had deteriorated. Best-corrected visual acuity (BCVA) was 1.2 in his right eye and 0.8 in his left eye, which were measured with a Japanese standard decimal visual chart. Funduscopic examination revealed irregular dark red lines that radiated toward the retinal periphery in both eyes, and the patient was diagnosed with AS. In the left eye, there was choroidal neovascularization (CNV) with SRH in the macular region. There was no pseudoxanthoma elasticum in the patient. He received photodynamic therapy and CNV regressed. One year after his first visit, CNV was observed with serous retinal detachment (SRD) in the right eye. The patient received intravitreal injection of bevacizumab and ranibizumab, and the SRD was resolved. Six years after the first visit, new streaks with SRH appeared in both eyes. Multiple thin SRH seemed to follow the path of the new streaks, which radiated toward the retinal periphery or extended toward the upper periphery from CNV. Three months later, new breaks became apparent when SRH was completely absorbed.
