概要 Objective/Purpose
Postoperative intraocular pressure after combined viscocanalostomy (VCS), phacoemulsification and aspiration (PEA) with intraocular lens implantation (IOL) has been reported as approximately 15 mmHg one to two years after surgery. This study retrospectively examined long-term postoperative outcomes.
Materials/Patients
We evaluated 47 cases (52 eyes) of primary open-angle glaucoma that underwent combined VCS and PEA + IOL at Kansai Medical University Takii and Hirakata Hospital from October 2000 through December 2007. These patients were followed up at least 1 year after operation. Mean age was 70.6 ± 8.7 years and mean postoperative observation period was 53.1 ± 27.7 months.
Methods
Changes in intraocular pressure, drug score (eye drops: 1 point per one drug, oral administration: 2 points per one drug) and intraocular pressure control rate were investigated, retrospectively.
Results and Conclusion
Mean preoperative intraocular pressure was 18.1 ± 3.3 mmHg. Mean intraocular pressure values at one, two, three, four and five years postoperatively were 15.0 ± 2.9 mmHg, 14.9 ± 3.3 mmHg, 14.4 ± 3.8 mmHg, 14.2 ± 2.2 mmHg and 15.5 ± 2.8 mmHg, respectively. The intraocular pressure control rate below 15 mmHg was 39.4% at ten years postoperatively. Mean preoperative drug score was 1.6 ± 1.0, while mean values at one, two, three, four and five years postoperatively decreased to 0.9 ± 0.9, 1.0 ± 0.9, 1.2 ± 1.0, 1.3 ± 1.0 and 1.3 ± 1.0, respectively. Two eyes required additional surgery involving trabeculotomy at 36 months for one eye and at 56 months postoperatively for the other eye.
The technique of combined VCS and PEA + IOL for primary open-angle glaucoma can achieve stable long-term postoperative intraocular pressure reduction.
