発表タイトル Analysis of Cytokines and Chemokines levels in CSF discriminates MS with and without Red Flags

概要 [Objective] There are atypical clinical and radiological findings regarded as warning signs during diagnosis of Multiple sclerosis (MS) and these signs are called Red flags (RFs). However, the immunological profiles of MS patients with RFs have not been investigated. We focused on the profiles of cytokines and chemokines in cerebrospinal fluid (CSF) of MS patients and examined whether these indicators could distinguish MS with RFs from without RFs. [Methods] A total of 35 patients satisfying the McDonald 2010 criteria were enrolled in this study. Among them four patients with anti- myelin oligodendrocyte glycoprotein antibody were excluded. 11 MS patients showed clinical and /or radiological RFs. We analyzed 13 cytokines and 13 chemokines in CSF of MS patients and those of 12 seropositive neuromyelitis optica (NMO) patients. [Results] First, we confirmed the difference of CSF profiles between MS without RFs and NMO. The levels of interleukin (IL)-12, B-cell activating factor (BAFF), a proliferation inducing ligand (APRIL), and CC- Chemokine Ligand (CCL) 2 were significantly higher in NMO than MS without RFs. MS without RFs showed higher levels of CCL3 and CCL5 than NMO. Then we used discriminant analysis for cytokines and chemokines levels in CSF among three groups: MS with and without RFs and NMO. This discriminant analysis model distinguished three groups with 93% accuracy. [Conclusion] Extensive analysis of cytokines and chemokines in CSF revealed immunological differences between MS with and without RFs.
