概要 | A Japanese man manifested bradykinesia of his left arm and leg at the age of 59, followed by propulsion and freezing of gait. He was diagnosed as Parkinson’s disease, and the symptoms were ameliorated by treatment with levodopa and dopamine receptor agonists. His motor functions were well-controlled and stable for 9 years, then bradykinesia and gait disturbance gradually deteriorated despite appropriate medications. Cognitive impairment and vertical gaze palsy developed subsequently, and he died of hepatic cell carcinoma at age 69. The fixed brain weighed 1250 grams. The globus pallidus (GP) was mildly atrophic. The discoloration of the substantia nigra (SN) was conspicuous. Microscopically, neuronal loss was marked in the subthalamic nucleus (STN), moderate in the GP and the SN, and mild in the dentate nucleus. AT8 immuno-positive neurofibrillary tangles and neuropil threads were observed in the pontine and dentate nuclei, striatum, oculomotor and trochlear nuclei, GP, SN, and STN, whereas only a few tufted astrocytes were identified. Clinicopathologically, our case is consistent with a diagnosis of “PSP-parkinsonism”. |
