概要 | (Purpose) Neovascular glaucoma (NVG) is one of the secondary glaucoma and it’s a very serious stage of disease, which is sometimes caused by proliferative diabetic retinopathy(PDR). When we treated patients of PDR with NVG secondary, we performed pars-plana vitrectomy (PPV) , combined with trabeculectomy (TLE). But TLE is difficult to apply in the cases with massive neovasucularization in the anterior segment of the eyes. The purpose of this study was to determine the IOP prognosis to PDR patients after treatment with PPV, combined with TLE after intravitreal bevacizumab(IVB).(Materials and Methods) We retrospectively reviewed the clinical records of NVG secondary to advanced PDR patients who received PPV+TLE during the period of September 2000 and January2008 at Kansai Medical University Hospital. IVB(1.25mg/100μul) was enrolled 3-6 days preoperatively. The patients were followed for more than12months. We studied visual acuity and IOP measurements at the initial visit, after IVB(before operation) ,3,6,9 and 12 months after operation. The success rates of surgery were determined by the Kaplan-Meier life-table analysis. Success was defined as IOP≦21mmHg at each examination. (Results)Ten eyes of 10 cases (5 males and 5 females; mean age , 56.6 ± 4.4 years(average ± standerd deviation (SD)) were recruited. The mean IOP before IVB was 41.6 ±9.2mmHg. In all eyes, iris and angle neovascularization regressed markedly after IVB(Fig1), however, the IOP remained elevated (mean IOP,39.4 ± 10.4mmHg). The mean IOP after surgery were 14.4± 3.8mmHg at 6 months ,and13.8± 4.2mmHg at 12 months. ・・・ |
