概要	[Purpose] To report the anatomical and visual outcome of primary vitrectomy with air tamponade to repair primary rhegmatogenous retinal detachment (RRD). [Patients and method] We examined the records of consecutive 10 cases that underwent primary vitrectomy for RRD with air tamponade, excluding cases with inferior retinal breaks. All patients were followed at least 3 months with an average follow-up of 6 months. Reattachment of the retina and visual outcome were compared to the results of previous cases that underwent primary vitrectomy for RRD with gas tamponade in our hospital. [Result] Reattachment was achieved in 10 of 10 eyes (100%) with a single operation. The average initial visual acuity was 0.3, and the average final visual acuity was 0.9 in 3 months. The postoperative complications such as macular pucker did not accept. There was no significant difference between primary vitrectomy with air tamponade and gas tamponade for the repair of primary RRDs. [Conclusion] Primary vitrectomy with air tamponade is a safe, effective method for the repair of primary RRD with superior retinal breaks. The anatomical reattachment rate and the visual acuity obtained with this technique appear to be as good as primary vitrectomy combined with long-acting gas tamponade.
